Does an inspection before buying actually catch the expensive stuff?

I’m about to buy a used car privately and everyone keeps telling me to get it inspected first, but I’m sceptical about how much an inspection really catches. A mate bought a car after an inspection and still had the gearbox fail within six months, so it’s not like it’s a guarantee.
